Our Services NHS Chronic Medication Service
Acute Dispensing
Patients on long-term medication may be offered a medication review by our Pharmacists  and  a long-term (24-48 week) prescription for their medication can then be issued by the patients surgery if your doctor agrees.

Who is this service for?

People with one or more long-term medical conditions that require medication.

A long-term condition is a health problem that is likely to last more than a year and needs on-going medical care.

Long-term conditions covered by the Chronic Medication Service include: Angina, Asthma, COPD, Chronic Pain, Diabetes, Epilepsy, Hypertension, Heart-Failure, Hypothyroidism, Hyperlipidaemia, Osteoperosis, Parkinson's Disease & Rheumatoid Arthritis.

The Chronic Medication Service is designed to help patients, doctors and pharmacists work more closely together to improve patient care.

You can register for the Chronic Medication Service if:

You are registered with a doctor's surgery in Scotland
and
You get prescriptions to treat a long term condition

What happens after I register?

Once you have registered for the service, your pharmacist will ask you about the medicines your are taking. This review will take place at a time convienient to you during the Pharmacy opening hours.

If there are any problems with your medicines (for example side-effects or difficulty using the medicine correctly) your pharmacist will work with your doctor to try and resolve the issue.

Once registered for the service your doctor may agree to precribe your medicines on a serial (long-term) prescription.  A serial prescription is like an automatic prescription but can last for up to 6 months. The prescription is kept in the Pharmacy and dispensed at 4 weekly intervals.

If you require your prescription early, or need extra supplies because you are going on holiday. Please contact the pharmacy in advance giving as much notice as possible.
